- Print a copy of the Life's Web Order Form.
(Please see instructions.)
- Using the Order Form, enter your Name,
Address, and Telephone Number.
- Enter your E-mail address if you want
us to confirm the receipt or shipment of your order. (See note below.)
- For each item you order, enter
the Photo Code, the Print
Size desired, the Number of Copies desired
per print size, the Price per Copy,
and the
Subtotal. (Enter
orders for different sizes of the same photo on different lines.)
- Add
your Shipping Cost, based on the largest print size in your order.
- Calculate
the Total Price for your purchase.
- Enclose a Payment with
your order. You may make the payment either by sending us a Money
Order in U.S. Currency, or by sending
us a Check, made payable
to Kendall and Susanne Corbin.
- Send the completed Order
Form and Payment to Life's Web at the address given above. (Please
note that if you
pay
us by
check, your
order will NOT
be filled
until after your check has cleared your bank account.)
NOTES:
- Obtain an Order Form by clicking the "Order
Form" button
above. Print that form on
your printer. (To print the
form, either select 80% from the Page Setup
option or print
the
form in Landscape Mode. To return to this
page after printing the form,
click on the "BACK" button at the upper right corner
of the Order Form before exiting from Life'sWeb.)
- Each photograph has a unique Code Number,
which appears on the individual Photograph Page. Include the code
number(s) of the photograph(s)
you want to
purchase.
- Enter your E-mail address if you wish
to be contacted by E-mail. We will NOT use your personal
information to send you advertisements, and we will NOT sell your personal
information
to other companies.
- You should allow up to two weeks for processing
and delivery of your order within the United
States. Delivery times outside the continental
U.S. vary significantly. All deliveries will be made by FedEX,
if possible.
